Charge Capture, Documentation Review and Coding

These services initiate what we at Instream refer to as our “ClaimPath”. Working with multiple providers in numerous facilities and locations, we have developed procedures that are flexible enough to accommodate different scenarios yet ensure proper data and charge capture for each client. We work with your hospital to gather patient insurance and demographic information, as well as clinical information related to your patients. We determine that we have all required supporting documentation in order to reduce unnecessary delays in data entry and claim submission, and we will provide you with concurrent feedback related to your documentation and charge records.
